Health History and Consent:

It is important that you honestly and accurately disclose any injuries,
surgeries, illnesses, or contagious conditions, etc..., on the
Health History
Form you will be given to fill out prior to your treatment. Massage would be
contraindicated, (not recommended), when certain conditions
or illnesses are present in the body.

I may decline to treat you and/or may refer you to your physician, for your own
safety, if any health concerns arise, or if a condition needs to be diagnosed
and/or cleared by your physician prior to receiving massage therapy.
Massage therapy is not a replacement for medical treatment.
Massage therapists are not able to diagnose illness,
prescribe medication, or perform manipulations of any bones.

By signing your Health History form, you are giving consent to receive
massage therapy. Remember, it is your choice to have massage therapy, you
may choose to end the session at any time.

Behavior and Conduct:

Massage Therapy is non-sexual, non-erotic contact between the massage
therapist and the client. Any behavior or discussion of a sexual or erotic nature
will result in immediate termination of the massage session.

You will be draped throughout your massage treatment; only the area that is
being treated will be un-draped.

A full body massage
does not include massaging of any breast tissue or genital
areas; these areas will
always remain draped throughout your
massage treatment.

If at any time during the massage you feel that a technique or treatment is
invasive or uncomfortable, please inform me immediately.

You may choose to terminate the session at any time.

Payment:
Payment is expected in full at the time of your treatment by credit card, cash or
room charge if you are a guest of the hotel.
